Act 4 of Path of Exile 2 introduces several optional objectives that can be easy to ignore while following the main storyline. Tribal Medicine is one of them, and its main item, the Shark Fin, deserves attention because it occupies a 2x2 inventory space and cannot be discarded normally. Players who are busy collecting equipment and POE2 Currency may overlook this small quest until their inventory becomes crowded. Fortunately, obtaining the Shark Fin and completing the quest is relatively simple once the correct locations are understood.
The quest is also worth completing because its reward changed significantly with version 0.5, The Return of the Ancients. Instead of the older skill gem reward, players now receive a choice between two permanent defensive bonuses. Since the selected bonus cannot be changed afterward, completing the objective is not simply about clearing an unwanted inventory item. It is also an opportunity to strengthen your character according to the defensive mechanics of your build.
Start the Search on Whakapanu Island
The Shark Fin comes from Whakapanu Island, specifically from the coastal area known as the Shark Pit. This optional location is home to the Great White One, the boss you need to defeat for the quest item.
The encounter itself is straightforward. Enter the Shark Pit, defeat the Great White One, and collect the Shark Fin after the fight. Because this boss is optional, players who concentrate exclusively on the main objectives can easily move through Act 4 without visiting the area.
After obtaining the item, keep it in your inventory until the quest has been completed. Although the Shark Fin has no combat purpose, its inventory size makes it inconvenient to carry around for too long. Turning it in promptly gives you more room for weapons, armour, crafting materials, and other useful loot.
Locate Kaimana During Normal Progress
Kaimana is the NPC who accepts the Shark Fin. If your Act 4 progression has not advanced beyond the relevant stage, you can normally complete the quest in Ngakanu.
Take the boat to Ngakanu and enter the village. Continue toward the far end of the settlement, where Kaimana can be found. Approach her while carrying the Shark Fin and interact with her to hand over the quest item.
For players following the campaign at a steady pace, this is the most convenient method. However, Act 4 contains story developments that can change the condition of certain areas. If you have pushed the main storyline too far, Ngakanu may become corrupted, preventing the normal hand-in from working.
Complete the Story When the NPC Moves
If Kaimana cannot be found in Ngakanu, the quest is not permanently blocked. Rather than repeatedly searching the corrupted settlement, continue progressing through Act 4.
The alternative route requires you to complete the act and defeat its final boss. Once the main storyline is finished, return to Kingsmarch. Kaimana will then appear south of the waypoint, on the second catamaran near the chieftain.
Speak with her at this location while the Shark Fin remains in your inventory. She will accept the item and allow you to finish Tribal Medicine. This route is particularly useful for players who collected the fin earlier but delayed the side quest until after advancing through the main story.
Examine Kaom's Lesson
The first permanent reward is Kaom's Lesson. It provides 30% increased Armour, Evasion, and Energy Shield, making it a flexible defensive option for many different character setups.
Its greatest strength is versatility. Builds that combine multiple defensive layers can benefit from all three bonuses rather than relying on a single protection system. Characters using both armour and evasion, for example, can gain meaningful value from the reward as their base defensive statistics increase.
Kaom's Lesson is therefore a practical choice for players who prefer broad defensive improvements rather than a bonus tied to one particular mechanic.
Consider Rakiata's Lesson
Rakiata's Lesson is the more specialized alternative. It allows 15% of Armour to also apply to elemental damage mitigation, grants Deflection Rating equal to 12% of Evasion Rating, and increases the speed at which Energy Shield begins recovering by 12%.
This combination can work especially well for characters with heavy armour investment but less elemental protection. It can also suit builds that actively use Deflection or rely on Energy Shield returning quickly after taking damage.
Neither reward is automatically better for every character. The right choice depends on the defensive mechanics already included in your build and the role each layer plays during combat.
Finish the Quest Before Moving On
Tribal Medicine is a small Act 4 side quest, but its permanent reward makes it more meaningful than its simple objective suggests. Defeating the Great White One, finding Kaimana, and choosing the appropriate defensive bonus can provide useful value before you move into more demanding content. Since the reward cannot be freely changed later, taking a moment to match it with your build is worthwhile. After the quest is finished and your inventory space is restored, you can focus on improving your equipment and preparing resources such as POE 2 Exalted Orbs for the challenges ahead.
